Requirements for Making Claims

Contact your HR for help to provide the following:

  • Completed Claim Form
  • If Natural Death, obtain Death Certificate and the Medical Certificate of cause of death of the employee.
  • In case of Accidental Death, obtain Police Report as well.
  • In the case of Disability, medical report is required to confirm the level of disability.
  • For Critical Illness, evidence of diagnoses of named Critical Illness.
  • Receipts for Medical Expenses relating to accidents, if any.
  • Copy of Ghana Card
  • Duly Completed Claim Form
  • Labour Sheets 1 & 2
  • Computation of Compensation from the Labour Office
  • Medical Receipts, if any
  • Confirmation of Wages/Salaries of Employee
  • Copy of Ghana Card
  • Duly Completed Claim Form
  • Police Report (if applicable)
  • Letter of demand or writ of summon (if any)
  • Incident Report
  • In case of Accidental Death, obtain Police Report as well.
  • In the case of Disability, medical report is required to confirm the level of disability.

  • Stock books indicate the value of stock held before loss
  • Assets register to show evidence of Assets own
  • In case of Fire, Fire Service Report
  • Police report, in case of theft or accident
  • Pictures of damaged/damaged items
  • Duly Completed Claim Form

  • Detailed client information to assess financial loss.
  • Correspondence related to the claim (emails, letters, legal documents)
  • Identities of the claimant and any other involved parties
  • Address proof of the insured location

Requirements for Motor Accidents Insurance Claims

  • Motor Accident Report Form
  • Driver’s License (Front & Back)
  • Estimate of Repairs
  • Pictures of the Damaged Vehicle (Show Vehicle number)
  • Police Report or Police Extract (If Applicable)
  • Original Documents of the Vehicle (In case of total loss claim)
  • Any document deemed necessary by the Insurance Company
  • Motor Fire Report Form
  • Driver’s License (Front & Back)
  • Estimate of Repairs
  • Pictures of The Damaged Vehicle (Show Vehicle Number)
  • Police Report (If Applicable)
  • Fire Service Report
  • Original Documents of the Vehicle (In case of a total loss)
  • Any document deemed necessary
  • Motor Accident Report Form
  • Driver’s License (Front & Back)

Third Party Claimant:

    • Estimate of Repairs
    • Pictures of the Damaged Vehicle (Show Car Number) or Property
    • Police Report
    • Letter of Claim (Satisfaction Letter If Repaired)
    • Any document deemed necessary after all documents
    • ID of the Third-Party claimant

Insured:

  • Motor Accident Report Form
  • Photocopy of Driver’s License (Front & Back)

Third Party Claimant:

    • Police Report
    • Medical Report
    • Medical Receipts
    • Letter of Claim
    • Affidavit
    • Two Passport Pictures (Endorsed by Medical Officer)
    • Pictures of The Injury
    • Surname and other names (I.D)
    • Date and place of birth
    • Profession or occupation and employer’s address (if any)
    • Income and relevant documents in proof of income, where necessary
    • Particulars of dependent(s) at the time of accident (if any)
    • The victim’s contact address and Phone number
    • Any document deemed necessary after all documents

Additional information and documentation to be Submitted by Claimant(s) Other than the Victim

  • Surname and other names of the claimants (I.D)
  • Date and Place of birth of the claimant(s)
  • Surname and other names of the victim
  • Relationship with the victim
  • Profession or occupation of the claimant(s) and employer’s address (If any)
  • A description of injuries sustained, financial loss incurred and any documents in support
  • Claimant(s) contact address and phone number
  • Two (2) passport-size photographs each of the claimant(s) and in the case of a minor, the photographs of the next friend
  • Identity card of the claimant(s)

Insured:

  • Motor Accident Report Form

Third Party Claimant:

    • Police Report
    • Medical Cause of Death Report or Coroners Report
    • Medical Receipts (If Any)
    • Letter of Claim

  • Two Passport Pictures (Endorsed by Public Notary)
  • Death Certificate
  • Letters of Administration (Not Applicable for Minor)
  • *Death Certificate of the Victim or Burial Permit or Postmortem Report
  • *Letters of Administration or proof of probate or Registrars Certificate
  • *Any identity card of Claimant{s)
  • *Relationship with the victim and identity cards of the beneficiaries
  • *Two (2) passport size photographs of the Administrator(s)/ claimant{s)
  • For the rural dwellers who may not be able to provide the above documents (*), a report from the Chief or religious leader and Statutory declaration, can be provided.
